Do I need to complete all my intake forms to schedule an appointment for my clinical care membership?

While you can schedule an appointment without completing your intake forms, we strongly recommend filling out your pre-visit questionnaires and any additional intake forms at least two days before your appointment. This allows your clinician to prepare and ensures you get the most out of your time together.

You can find all required forms on the home page of your My Parsley Health portal starting 7 days before your scheduled visit.
