Where can I find details about my upcoming visit?

At least two days before your visit, refer to the main details of your visit on the Upcoming page of My Parsley Health.  There you can see the date, time and location details of the visit, so you will know where and how to meet with your provider.  

Additionally, be sure to complete your pre-visit questionnaire located beneath your scheduled visit on the same Upcoming page of your portal.  

Depending on the visit, you will need to fill out a Parsley Symptom Index (PSI) or a Visit Prep Form, two days prior to your visit.  Lastly, you’ll also receive reminder emails with these same details, so be sure to check and read your emails from Parsley Health.
