What if I need more time to fill out my questionnaire?
Not a problem! Our questionnaires are long and comprehensive and we understand that you might not know all the answers right away. Some of the questions may require you to reach out to your relatives for the answer(s). Because these questionnaires require a bit of time, we have set them up to automatically save each time you enter a response. That way, you can always return to where you left off if you need more time to fill out the questionnaire.
However, it is important to note that once you submit your questionnaire to the Care team, you will not be able to revise it again. So, it would be best to take your time filling out your questionnaires and only submit them once you are totally comfortable with your answers.
