I had filled out my intake forms, but I'd like to change some answers and resubmit. How do I do this on my Longevity Labs plan?

At the moment, the My Parsley Health portal does not allow members to access or edit forms that have already been submitted. To fill out and submit a new questionnaire, click on your profile in the upper right corner and “Intake Forms” in the navigation dropdown.

Next, click the 'New Form' button in the upper-right, and select the form(s) you'd like to resubmit to your care team.  

Please note that all questionnaires on the My Parsley Health portal automatically save when you complete a question. To complete the form at another time, you can simply leave the page and return to where you left off later. Only submit your questionnaire when it is fully ready to be sent to your care team for review.
